Certificate of designation forms
The places where scientific procedures are carried out must be licensed under a Certificate of Designation.
A revised certificate of designation application form is available for use with new applications and amendments to existing certificates.
The revised form is intended to simplify the process of changing the details of named persons, and provide flexibility for the use of approved areas, reducing the details recorded on the schedule of premises. The greater flexibility has potential to considerably reduce the need for amendments. On the revised form only one signature and no changes to the schedule are needed when changing details of named persons.
We would in particular urge those who request more than one amendment per year to use the revised form. However there is no requirement to change to the revised form, and the previous form and notes remain to facilitate minor amendments to existing certificates, particularly where conversion to the new format would be unnecessarily burdensome.
